Job Summary

We are seeking a Licensed Practical Nurse (LPN) to join our team as a Hospice Care Specialist. In this role, you will provide safe and respectful nursing care to patients with complex or unpredictable outcomes, working collaboratively with the RN/RPN and other members of the interdisciplinary care team. Your expertise will be essential in developing and implementing individualized care plans that support the abilities, preferences, and routines of our patients.

Responsibilities
  • Conduct head-to-toe assessments, monitor and recognize abnormal signs or symptoms, and communicate with the RN/RPN as needed.
  • Support and partner with the RN/RPN in the care of patients with complex or unpredictable outcomes.
  • Communicate with physicians regarding assigned patients and carry out verbal and/or written orders as required.
  • Administer medications, including narcotics, according to established procedures, and monitor medication effects.
  • Respond to incidents such as Codes, injuries or falls, hazardous spills, aggressive behavior, and fire alarms, reporting incidents and filling out appropriate forms in accordance with established procedures.
  • Assist with the orientation of new staff and student preceptors by performing duties such as demonstrating work procedures, proper use/location of equipment, sharing clinical knowledge, using effective communication, and acting as a positive role model.
Requirements
  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training, and experience.
  • Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
  • Current basic cardiac life support certification.
