Job Overview:

About the Role:

The successful candidate will be responsible for coordinating and overseeing all aspects of resident-focused care delivery. This includes providing direct supervision, leadership, and education to other healthcare team members. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Responsibilities:

  • Develop and modify plans of care in response to residents' individualized health needs
  • Liaise with other healthcare personnel, agencies, and families
  • Maintain good relationships with medical/support/home staff, residents, families, and the general public
  • Supervise all activities on the unit, taking charge as required
  • Monitor performance and complete performance evaluations for PSWs and RPNs
  • Direct, assign, and evaluate the work of PSWs
  • May be in charge of the home as required, utilizing leadership skills to identify problems and problem-solving on the unit
  • Ensure care delivery is consistent with the resident Bill of Rights
  • Required to understand quality indicators used by the home and participate in their improvement
  • Order and ensure nursing supplies and equipment are properly and efficiently used
  • Actively listen to resident and family concerns, taking appropriate actions to resolve issues
  • Conduct unit rounds at the beginning and end of shift
  • Process physician orders, administer medications and treatments, and document errors and omissions
  • Assess changes in health status and report them to SDM or POA and the interdisciplinary team
  • Complete healthcare assessments and develop/update plans of care for each resident

Qualifications:

  • Current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse or equivalent in another Canadian jurisdiction
  • Basic CPR certification
  • Experience in long-term care settings is an asset
  • Ability to work effectively with interdisciplinary teams and communicate well with residents, families, and staff
  • Well-developed written and oral communication skills
  • Knowledge of RAI-MDS – AIS certification (if not already obtained)
  • Ability to provide evidence of being focused on people's feelings when interacting and turning tasks into positive social interactions
  • Must meet legal requirements and promote individuals' rights while reducing elements of risk
  • Must provide a current Vulnerable Sector Check and TB testing results (as per Public Health requirements)
  • Two supervisory references are required
  • Covid-19 vaccination is a condition of employment
