Patient Care Coordinator

We are seeking a Registered Practical Nurse (RPN) to provide quality nursing care to patients in various settings. In this role, you will work collaboratively with healthcare professionals to ensure coordinated care and provide education and support to patients and their families.
Key Responsibilities:- Assess patients' needs and develop care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' vital signs and report changes in condition.
- Collaborate with healthcare professionals for coordinated care.
- Provide education and support to patients and families.
- Valid RPN license in Canada.
- Completion of a recognized Practical Nursing program.
- Previous experience in a healthcare setting is preferred.
- Strong communication and interpersonal skills.
- Ability to work in a fast-paced environment.
