Job Summary

RT Medical is seeking a skilled Registered Practical Nurse to join our team as a Home Care Specialist. In this role, you will provide high-quality care to individuals in their homes, working collaboratively with patients, families, and healthcare professionals to develop and implement personalized care plans.

Key Responsibilities
  • Conduct thorough assessments of patients' needs and develop comprehensive care plans
  • Provide skilled nursing care, including wound care, IV medications, and catheter care
  • Collaborate with patients, families, and healthcare professionals to ensure seamless care transitions
  • Travel to patients' homes, clinics, and other healthcare settings as needed
  • Document patient care and progress in a timely and accurate manner
Requirements
  • Diploma in Registered Practical Nursing or equivalent
  • Current registration with the College of Nurses of Ontario
  • Valid Standard First Aid and Level C CPR certification
  • Up-to-date immunization records, including a two-step tuberculosis test within the last 6 months
  • Clear criminal background checks and Vulnerable Sector checks
