The LPN Medication Care Manager is responsible for administering medication and treatments in a safe organized manner. This role requires strong communication skills and the ability to work effectively with residents, families, and healthcare professionals.


Responsibilities

- Administer medication and treatments according to established protocols and procedures.
- Monitor residents' medication regimens and adjust as needed.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Communicate effectively with residents, families, and staff regarding medication administration and resident care.


Qualifications

- Current licensure as an LPN in the state of practice.
- Minimum 1 year of experience in a long-term care setting or equivalent combination of education and experience.
- Strong knowledge of medication administration and management practices.
