Primary Care Social Worker

The Fraser Health Authority is seeking a skilled and compassionate Primary & Community Care Social Worker - Indigenous to join our team in Hope, Agassiz, Harrison Hot Springs, and Kent. This full-time position will provide comprehensive services and therapeutic clinical counseling to clients and their families who identify as Indigenous.
The ideal candidate will have a Bachelor's degree in social work, two years' recent related experience in a complex health care environment, and relevant experience in an indigenous environment/community. They will be registered with the BC College of Social Workers and possess a valid BC Driver's License.
We are looking for a candidate who can work effectively in a multicultural environment, as part of an interdisciplinary team, and communicate effectively both verbally and in writing. The successful candidate will also have the ability to educate and provide consultation to patients, families, and healthcare professionals.
Responsibilities
- Conduct comprehensive social work and psychosocial assessments by interviewing the client and/or their family, obtaining relevant information, gathering social data, and interpreting the data using a variety of tools.
- Provide comprehensive therapeutic services by exploring the psychosocial response to identify problems, personal concerns, treatment objectives, and facilitate change in coping style, behavior, attitude, and feelings.
- Collaborate with the interdisciplinary team in client care planning by interviewing the client and/or family to assess and determine care plan and type of service required.
- Support a seamless approach to care and service delivery across the healthcare system by participating in the admission and discharge process of clients in acute care and community.
Qualifications
- Current full registration with the BC College of Social Workers.
- Two (2) years' recent related experience in a complex health care environment such as a community or home health setting, plus relevant related experience in an indigenous environment/community; or an equivalent combination of social work education, training, and experience.
- Valid BC Driver's License and access to personal vehicle for business-related purposes.
