**Job Summary**

We are seeking an experienced Advanced Practice Nurse to join our team at Health Careers in Sask. As a Nurse Practitioner - Community Care Specialist, you will be responsible for providing primary care services to patients in the community setting.

Key Responsibilities:

  • Provide comprehensive health assessments and develop treatment plans for patients with acute and chronic conditions
  • Order diagnostic tests and prescribe medications as needed
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
  • Develop and implement community-based programs to promote health and prevent disease

Requirements:

  • Master's degree in Nursing or related field
  • Two years of experience as an Advanced Practice Nurse
  • Valid registration with the College of Registered Nurses of Saskatchewan (CRNS)
  • Basic Cardiac Life Support Level C Certificate (BCLS)

Preferred Qualifications:

  • Experience working in forensic/clinical treatment with youth/families
  • Knowledge of conflict resolution techniques and leadership strategies
  • Ability to adapt to changing conditions and demonstrate empathy, trust, and respect in the provision of care
