Job Overview

Bayshore HealthCare is seeking a highly skilled and compassionate Home Care Manager to join our team in Etobicoke, Ontario. As a key member of our community health team, you will play a vital role in providing exceptional home care services to clients who wish to age in place.

Estimated Salary Range: $85,000 - $110,000 per year

About the Role

The Clinical Leader position is a hands-on community health role where collaboration with formal and informal care networks supports a client and family-centered home care model in your local neighborhood. You will have the opportunity to provide personalized care to clients while building relationships in the community.

Main Responsibilities

  • Develop comprehensive plans for clients and families using a holistic approach to help them achieve their aging in place goals.
  • Direct and monitor the delivery of all in-home services and daily functions of field staff, providing access to clinical advice, support, and resources.
  • Hire and train healthcare staff, supervise day-to-day performance, ensure exceptional interactions with prospective and existing clients, and provide ongoing coaching and performance management.
  • Manage expectations with family members and clients, ensuring a client and family-centered approach to care planning.
  • Adapt care to changing client needs, including occasionally working with the interdisciplinary team for management of acute illness, falls, and other immediate challenges.

Requirements

  • A current and valid Registered Nurse (RN) or Registered Practical Nurse (RPN) registration with the College of Nurses of Ontario (CNO).
  • Minimum of five years of recent experience, preferably in a community setting, with at least two years managing others.
  • Current and valid CPR certification.
  • Experience in community healthcare and local community resources.
  • Experience working with the geriatric population and their families is an asset.
  • A reliable vehicle and willingness to travel to visit clients within the community (approximately 60% in the field). Vehicle allowance is provided.
