Job Summary

Island Health is seeking a highly skilled and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in providing emotional support and guidance to patients and their families, helping them navigate complex healthcare systems and access necessary resources.

Key Responsibilities
  • Assessment and Intervention: Conduct thorough assessments of patients' social and emotional needs, and develop and implement effective intervention plans to address these needs.
  • Case Management: Coordinate and facilitate communication between patients, families, healthcare providers, and community resources to ensure seamless care and support.
  • Education and Consultation: Provide education and consultation to patients, families, and healthcare providers on topics such as crisis counseling, loss and grief, and mental health issues.
  • Community Resource Development: Develop and maintain relationships with community resources, such as social services, health and welfare programs, and support groups, to ensure patients have access to necessary services.
  • Documentation and Reporting: Maintain accurate and comprehensive records of patient interactions, interventions, and outcomes, and provide regular reports to healthcare teams and management.
Requirements
  • Education: Master's degree in Social Work from a recognized university.
  • Experience: Two years of recent experience in a healthcare setting, preferably in a social work or counseling role.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and healthcare teams. Strong assessment and intervention skills, with the ability to develop and implement effective plans to address complex social and emotional needs.
