Job Title: Community Rehabilitation Worker

Blessing Home Care and Group Home Ltd is seeking a skilled Community Rehabilitation Worker to join our team. As a Community Rehabilitation Worker, you will play a vital role in supporting individuals with various needs, including those with intellectual, learning, and mental health disabilities.

Key Responsibilities:
  • Prepare and maintain accurate reports and case histories for clients.
  • Conduct social research and assessments to identify client needs and develop personalized plans.
  • Implement life skills workshops and behavior management programs to support clients in achieving their goals.
  • Supervise activities of clients living in group homes and half-way houses, ensuring their safety and well-being.
  • Liaise with other social services agencies and healthcare providers to ensure comprehensive support for clients.
  • Assist clients with special needs, accompanying them to appointments and social outings as needed.
Requirements:
  • 1-2 years of experience in a related field, preferably in community rehabilitation or social work.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse client groups.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Physical stamina to perform tasks that may require lifting up to 23 kg (50 lbs).
