As a Patient Care Coordinator, you will be responsible for coordinating patient care services in a home or community setting. This role requires strong clinical and communication skills, as well as the ability to work effectively with clients, families, and healthcare team members.
Responsibilities:
- Clinical Practice: Utilize the nursing process and standards of care to provide high-quality patient care. Perform pronouncements, monitor and document DRN information, work independently in the community, complete point-of-care assessments, and implement client care plans.
- Collaboration and Communication: Engage with clients, families, and healthcare team members to ensure effective coordination of care. Participate in quality processes, complete necessary documentation, and utilize technology to communicate with others.
- Leadership and Mentoring: Demonstrate leadership skills, motivate and help others grow, and participate in corporate quality initiatives.
Requirements:
- Education: BSc in Nursing, required
- Registration: Current and unrestricted registration with College of Nurses of Ontario, required
- Experience: Previous experience in Home or Community nursing environment, preferred; previous experience with surgical discharge clients, preferred
- Certifications: Basic cardiac life support (BCLS) education and/or experience, required; IV medication administration education and/or experience, preferred
