Our team is dedicated to helping clients maintain their independence and quality of life. We are seeking a skilled and compassionate individual to join our team as a Community Health Worker.

The successful candidate will provide personal care and daily living support to clients, while promoting maximum independence and dignity.

Key Responsibilities:

  • Assist clients with activities of daily living, such as bathing, dressing, and grooming
  • Provide household support to maintain a safe and healthy environment
  • Prepare meals in accordance with designated plans
  • Assist with mobility and transfers
  • Observe and report changes in client behavior and condition
  • Perform delegated tasks according to training and procedures

This is a Relief Part-Time opportunity until the return of the incumbent. The shift types for this position are rotating days and evenings with hours of work from 07:00 to 14:30, 07:00 to 15:30, 15:00 to 23:00 hours.
