Job Title: Social Worker - Mental Health Outpatient Program

Job Summary:

We are seeking a highly organized and skilled Social Worker to join our interprofessional team at St. Joseph's Care Group. The successful candidate will work with clients to provide strength-based care, including accessibility, accountability, best practices, and evaluation.

Key Responsibilities:

  • Provide assessments, treatment, and individual and group counseling to clients with serious and persistent mental illness.
  • Develop and implement treatment plans that incorporate evidence-based therapies, such as solution-focused therapy, EMDR, CPT, CBT, DBT, and group therapies.
  • Collaborate with the interprofessional team to provide patient-centered care and support.
  • Participate in decision-making processes and contribute to the development of policies and procedures.
  • Stay up-to-date with relevant legislation, such as the Mental Health Act, Health Care Consent Act, and PHIPA.

Requirements:

  • Masters of Social Work Degree and current registration with the OCSWSSW.
  • Knowledge and experience with referral processes and triage, decision guide processes, and system navigation.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
  • Ability to utilize technology to effectively document in electronic medical records and retrieve relevant information.
  • Knowledge of Indigenous worldviews, cultural practices, and understanding of the impact of colonization on Indigenous populations (an asset).

Condition of Employment:

  • This position is conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated.
