Transforming Healthcare Delivery as a Senior Speech Therapist

CBI Home Health is seeking a dynamic and compassionate Senior Speech Therapist to join our innovative CBI Home Health Durham OHT Leading Practice program. As a key member of our interdisciplinary team, you will play a vital role in delivering exceptional care to diverse individuals and communities.

About the Role
You will be responsible for assessing and implementing speech language pathology interventions, leveraging virtual and in-person delivery methods to provide personalized care. Your focus on person-centered care, combined with your expertise in dysphagia management, communication, and swallowing assessments, will empower patients to reclaim their independence and thrive in their daily lives. Responsibilities
* Assessment and implementation of SLP interventions (i.e., swallowing assessments, dysphagia management, communication, etc.) in a home-based setting
* Use of a collaborative and interdisciplinary team-based approach to care and services
* Support a holistic and restorative approach to care (i.e., enhanced stroke pathway; cognitive communication supports, etc.)
* Leverage digital health platforms to support care delivery, collaboration, documentation, and intervention

Requirements
* Registered as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
* Focus on person and family-centered care
* Comfortable delivering care both in person and using virtual methods/technology
* Passionate, empathetic, use good judgment, and apply critical thinking when making client decisions
* Excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
* Flexible and have demonstrated skills in critical thinking that can be applied to a variety of situations
