Job Title: Physiotherapist

Job Summary:

We are seeking a skilled Physiotherapist to join our team at Home and Community Care Support Services. As a Physiotherapist, you will be responsible for providing physiotherapy services to patients in their homes, schools, or community settings.

Key Responsibilities:

  • Provide physiotherapy services to patients to reduce, eliminate, or prevent physical disabilities/impairments and improve functional independence and safety.
  • Assess patients' physical abilities and develop individualized treatment plans.
  • Collaborate with healthcare professionals, case managers, and community organizations to ensure comprehensive care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Stay up-to-date with industry developments, best practices, and evidence-based research to provide high-quality care.

Requirements:

  • Registered Physiotherapist in good standing with the College of Physiotherapists of Ontario.
  • Strong knowledge of physiotherapy services, community resources, and healthcare systems.
  • Excellent communication, interpersonal, and collaboration skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and access to a vehicle.
  • Advanced oral and written proficiency in English is essential, French would be considered an asset.
