About the Job

We are seeking a skilled Social Worker to join our team at Extendicare Canada Inc. as a Resident Care Specialist. This is a full-time position in our Sault Ste Marie location.

Job Summary

The successful candidate will be responsible for providing social work services to residents and families in our long-term care home. This includes participating in the resident admission process, acting as a liaison with families and advocate for residents, and providing therapeutic services through individual and group counseling.

Responsibilities

  • Participate in resident admission process, including tours, admission, transfers, discharge, and occupancy tracking
  • Act as liaison with families and advocate for residents to ensure they are allowed and encouraged to exercise their rights
  • Participate in interdisciplinary team process of resident assessment, care planning, and evaluation to ensure services are planned according to individual resident and family needs
  • Provide therapeutic services through individual and group counseling for residents and families, document on resident's record, and evaluate effectiveness of these programs
  • Adhere to established department plans, policies, procedures, and quality assurance program to meet all regulatory and Extendicare standards
  • Provide in-service programs for home staff, residents, and families, encourage and support professional growth and development of staff
  • Maintain all required records, reports, statistics, and documentation for both residents and department in accordance with policies, procedures, and legislative requirements
  • Participate on committees related to resident care and operation of the home as required
  • Liaise and consult with community partners, company resources, and professionals in relation to resident care needs and department activities
  • Acknowledge and request referral to a Social Worker for areas beyond scope of practice
  • Act as Resident Council Assistant/Liaison
  • Assist with recruitment, screening, orienting, and recognizing volunteers
  • Other duties as assigned by the Administrator

What You'll Bring

  • Social Service Worker Diploma or Degree in Social Work is mandatory
  • Registration with The Ontario College of Social Workers and Social Service Workers (OCSWSSW) is mandatory
  • Previous experience in a long-term care setting preferred
  • Excellent communicator, well-organized team player with solid clinical skills in assessments and care planning, supportive therapy, crisis intervention, and grief counseling, preferred
  • Expertise in working with geriatric populations with specialization in Alzheimer's disease and other dementias, depression, responsive behaviors, and other psychogeriatric issues, preferred
  • Knowledge of applicable Ministry of Health and Long-Term Care legislation and standards, preferred
  • Knowledge of occupational health and safety practices, principles, and legislation, preferred
  • Computer literate with understanding of Microsoft Office Applications
