About the Job

This is an exciting opportunity for a highly motivated and experienced Nurse Case Manager to join our dynamic team at Bayshore HealthCare. As a key member of our organization, you will be responsible for providing comprehensive support to patients living with a dermatological disease.

Key Accountabilities

  • Develop and implement effective case management plans to ensure timely access to treatment for patients.
  • Collaborate with healthcare professionals, including physicians and pharmacists, to coordinate patient care.
  • Provide education and support to patients and caregivers on disease management and treatment options.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Manage and maintain accurate and comprehensive patient records.
  • Identify and address potential barriers to care, including financial and social determinants.
  • Stay up-to-date on the latest treatment options and research in dermatology.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
  • Maintain confidentiality and handle sensitive patient information with discretion.
  • Travel to healthcare facilities and community locations as required.

Requirements

  • Current RN or RPN registration.
  • Experience in patient support programs, preferably in dermatology.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong analytical and problem-solving skills.
  • Proficiency in electronic medical records and practice management software.
  • Ability to prioritize tasks and manage multiple responsibilities.
  • Valid driver's license.
