We are seeking a highly skilled and experienced Public Health Nurse to join our team at the Cree Board of Health and Social Services of James Bay (CBHSSJB). The successful candidate will be responsible for providing comprehensive nursing care to patients undergoing renal transplant, as well as coordinating with multidisciplinary teams to ensure holistic patient care.
Key Responsibilities- Assess patient needs and develop individualized care plans
- Coordinate with healthcare teams to ensure seamless care delivery
- Provide education and support to patients and their families
- Collaborate with community organizations to promote health and wellness
- Participate in quality improvement initiatives to enhance patient outcomes
- Bachelor of Science in Nursing degree
- Experience in hemodialysis and renal transplant nursing
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Fluency in English; fluency in Cree or French is an asset
