We're seeking a compassionate and skilled Health and Social Support Specialist to join our team at Njoyn. As a key member of our healthcare team, you'll be responsible for providing high-quality psychosocial support to patients and their families, helping them navigate complex social situations and make informed decisions about their care.
**Job Description:**
- Provide comprehensive psychosocial assessments to identify areas of strength and vulnerability for patients and families.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between services.
**Qualifications:**
- Bachelor of Social Work degree or equivalent.
- Current registration with the Provincial Association of Social Workers.
