Job Summary

The Personal Support Worker provides personalized support and homemaking services to clients and families within the framework of VON policies, procedures, and standards. The PSW collaborates with members of the healthcare team in meeting the needs of clients to attain an optimal level of independence and well-being.

Key Responsibilities
  1. Client Assessment: Observes and evaluates the client's or family's home, personal situation, and client and family care plan to determine if any need for change in the personal support care plan is required and whether the client's and family's needs have been met. Advises supervisor accordingly.
  2. Care Plan Development: Contributes to the development of the personal support care plan under the direction of the Personal Support Program Manager.
  3. Care Plan Implementation: Implements changes to the care plan under the guidance of the Manager for safety, liability, and financial reasons.
  4. Communication: Reports verbally to the immediate supervisor or Personal Support Program Manager any changes in the client's or family's personal support needs or home situation.
  5. Personal Support Tasks: Performs personal support tasks reflecting the needs of the client and family, including light cleaning, meal preparation, and shopping.
  6. Personal Care: Provides personal care as directed by the Personal Support Program Manager.
  7. Bedside Care: Provides simple bedside care, where required, under the direction of a nurse.
  8. Respite Care: Provides respite care to clients as required.
  9. Child Care: Supports clients' families by providing child care as required.
  10. Activity Planning: Organizes each day's activities, setting priorities for personal support and other activities under the direction of the Personal Support Program Manager.
  11. Documentation: Completes daily call checks, and other forms as required.
  12. Client Communication: Assesses the client's pattern of communication and ability to relate.
  13. Team Collaboration: Communicates with appropriate members of the health team regarding the management of the client's or family's needs.
  14. Compliance: Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and VON policies and procedures.
  15. Additional Duties: Other duties as assigned.
External and Internal Relationships
  1. Supervisor Relationship: Engages frequently with the immediate supervisor or the Personal Support Program Manager.
Education, Designations, and Experience
  1. Education: High school diploma.
  2. Secondary Education: Secondary school education is preferred.
  3. CCA Certification: CCA Certification is required in Nova Scotia.
  4. Personal Support Worker Certification: Must have certificate in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide.
  5. First Aid Certification: Must have a current Standard First Aid Certificate or CPR level C Certificate.
  6. Experience: Some household management experience and experience working with elderly or ill individuals or children.

  1. Communication Skills: Strong communication skills, both verbally and written.
  2. Client Interaction: Able to communicate effectively with clients and families in a respectful manner.
  3. Assessment Skills: Good assessment skills.
  4. Community Awareness: Awareness of community resources.
  5. Teamwork: Ability to work effectively in a multidisciplinary team or independently.
  6. Adaptability: Ability to adjust to working in different social and economic environments.
  7. Confidentiality: The capacity to adhere to strict confidentiality.
Other
  1. Transportation: Must have access to a personal vehicle for work and possess a current driver's license and proof of vehicle insurance.
  2. Criminal Record Check: A current and original copy of a satisfactory Criminal Records Check.
  3. Vulnerable Sector Search: A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required.
  4. Flexibility: Available to work flexible hours.
  5. Personal Protective Equipment: Must be able to wear Personal Protective Equipment (PPE).

Working Conditions and Physical Demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, and other movements that emphasize good body mechanics.
