Job Description:

As a Registered Practical Nurse (RPN) with ParaMed, you will play a vital role in delivering scheduled visits to patients in various settings. Your expertise and compassion will enable you to work effectively with patients of all ages, addressing their unique care needs and promoting independence.

Responsibilities:

  • Care for patient needs during scheduled visits, using appropriate assessment tools and techniques to assess individualized needs
  • Execute and continuously monitor care plans, engaging patients through a collaborative approach by building rapport and trust
  • Maintain and update professional patient records related to patient encounters and plans

Required Skills and Qualifications:

  • Registration with the CNO as a Registered Practical Nurse in good standing
  • Valid driver's license and access to a vehicle required
  • Experience in delivering community care is an asset
  • Reliability and flexibility, with willingness to travel to and from patient residences within a designated area
  • Passion for making a difference in your community
