About the Role

We are seeking a highly skilled Occupational Therapist to join our team at Fraser Health Authority. As an Occupational Therapist, you will play a vital role in providing high-quality patient care and promoting functional independence in our community-based settings.

Key Responsibilities
  1. Assessment and Intervention: Conduct thorough assessments of clients' cognitive, perceptual, and physical abilities to identify areas for improvement. Develop and implement personalized treatment plans to promote functional independence and occupational performance.
  2. Collaboration and Communication: Work closely with clients, families, caregivers, and other healthcare professionals to ensure seamless care coordination and effective communication.
  3. Education and Training: Educate clients and caregivers on independent living skills, occupational performance, and the use of environmental and equipment adaptations.
  4. Resource Coordination: Coordinate rehabilitative services with clients and other healthcare providers, including home care nursing, case management, and community resources.
  5. Advocacy and Support: Act as a client advocate, providing information and liaison services with funding agencies and community resources to ensure access to necessary support.
  6. Quality Improvement and Risk Management: Participate in quality improvement and risk management activities to ensure high standards of care and client safety.
  7. Documentation and Reporting: Maintain accurate patient records and reports statistics to meet legal and data requirements.
Requirements
  • Current full registration with the College of Occupational Therapists of BC and eligibility for membership in the Canadian Association of Occupational Therapists.
  • Two (2) years' recent, related clinical experience in a community or home health setting, or an equivalent combination of education, training, and experience.
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes.
