Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our High Complexity Care Team in Victoria PCN. As a key member of our team, you will provide comprehensive care to complex, resource-intensive patients in Urban Greater Victoria PCN Boundaries and First Nations communities.
Key Responsibilities:- Provide primary care to a minimum patient panel size, with a focus on complex, resource-intensive patients.
- Work collaboratively with the Physician Lead and other healthcare professionals to optimize team-based, patient-centred care.
- Develop and implement care plans that address the unique needs of each patient.
- Manage high-acuity and conflicting chronic disease, including medication management and planning around acute care utilization reduction.
- Current registration as a Nurse Practitioner with the College of Registered Nurses of British Columbia.
- Experience working with complex, resource-intensive patients in a primary care setting.
- Strong communication and collaboration skills, with the ability to work effectively in a team-based environment.
