We are seeking a highly skilled Registered Nurse to join our team at Peace Arch Hospital in White Rock, BC. As a Registered Nurse in our Acute Care for Elders unit, you will provide direct patient care to acutely ill elderly patients, working collaboratively with the interprofessional care team to deliver person-centered care.
Responsibilities- Assess patients within the acute care unit for elders, collecting information from various sources and making nursing diagnoses to determine the best course of care.
- Collaborate with patients and their families/caregivers to develop and implement care plans, promoting patient autonomy and dignity.
- Participate in care and discharge planning, ensuring effective communication with the interprofessional care team and Patient Care Coordinator.
- Coordinate nursing care, prioritizing tasks and delegating to support staff as needed.
- Collaborate with the interprofessional care team to identify and resolve issues, adjust care plans as required, and address changes in patient condition.
- Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance to support their health care goals.
- Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
To be successful in this role, you will require:
- Graduation from an approved school of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One (1) year recent related acute care experience, or an equivalent combination of education, training, and experience.
- Ability to complete initial and ongoing patient assessment, provide nursing care, and communicate effectively with patients, families/caregivers, and the interprofessional care team.
- Ability to promote person-centered care, integrate and evaluate data from multiple sources, and teach patients and families/caregivers about topics unique to the geriatric population.
