Vancouver Coastal Health (VCH) is seeking a Nurse Practitioner to join the Acute Home Based Treatment (AHBT) Team. This is an exciting opportunity for a dedicated healthcare professional to make a difference in the lives of patients and families in Vancouver.
Key Responsibilities
- Provide comprehensive assessments and diagnoses of patients, including prescribing medications and initiating treatment.
- Work collaboratively with a team of healthcare professionals to provide evidence-based care and develop population-based strategies to improve health outcomes.
- Develop and implement care plans that address the unique needs of patients and families.
- Participate in peer review and quality improvement activities to ensure high-quality care and patient safety.
Requirements
- Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
- Four (4) years of recent, and relevant clinical nursing experience.
- Successful completion of the Objective Structured Clinical Examination (OSCE).
