Vancouver Coastal Health is seeking a Community Health Worker to join our Home Support Team in Vancouver, BC.

Job Summary:

We are looking for a dedicated and compassionate individual to provide home support services to clients. As a Community Health Worker, you will work under the direction of the Supervisor/Coordinator/Manager or delegate to assist clients with varied levels of care, observe, and report significant changes in the client's presentation.

Responsibilities:
  • Provide home support services to clients, including assistance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
  • Collaborate with the healthcare team, clients, and their caregivers to maximize independence.
  • Motivate and encourage clients to participate in their care.
  • Provide input and feedback in the care planning process to the healthcare team.
Qualifications:
  • Graduation from a recognized Health Care Assistant Program.
  • Current/Active Registration with BC Community Health Worker and Care Aide Registry.
  • Experience interacting with persons with dementia, geriatric, and/or palliative population will be an asset.
  • Valid BC Driver's License.
