Salary

This full-time position offers a competitive salary of $26.00 per hour, with an estimated annual salary of approximately $54,080 based on 40 hours of work per week.

Job Description

The successful candidate will be responsible for leading and directing a team of Personal Support Workers to deliver high-quality client-focused service. This includes supervising, coaching, and supporting PSWs to provide exceptional care that meets client needs and expectations.

You will be responsible for assessing and arranging for initial and ongoing assessments of clients requiring unlicensed care and services as appropriate. You will also develop and/or ensure that care plans are available for clients as needed, visit clients as required, and update care plans accordingly.

Requirements/Qualifications

To be successful in this role, you must possess a minimum of 2 years of experience as a regulated health professional RN/RPN, preferably with 2-3 years of recent experience in a management position in a community setting. You must also have solid knowledge of principles, practices, and methods of community nursing and service delivery to clients.

You should be able to supervise a team of Personal Support Workers, have valid drivers' licenses, and evidence of current CPR certification. Additionally, you must have strong time management skills, confidentiality, and professionalism, as well as excellent communication skills to foster relationships with Home Care Staff and clients.

Responsibilities

Key responsibilities include:

  • Leading and directing a team of Personal Support Workers to deliver high-quality client-focused service
  • Assessing and arranging for initial and ongoing assessments of clients requiring unlicensed care and services as appropriate
  • Developing and/or ensuring that care plans are available for clients as needed
  • Visiting clients as required and updating care plans accordingly
  • Conducting training sessions and facilitating and monitoring training and educational opportunities for Personal Support Workers
  • Ensuring that all client concerns and complaints are addressed and documented in a timely manner
  • Demonstrating knowledge of Standards of Practice guidelines of Provincial regulating body
  • Communicating and working closely with Coordinators responsible for scheduling Personal Support Workers
  • Maintaining confidentiality of client, PSW, and corporate information
