Schedule Planning and Coordination RoleThis role involves assigning client visits to nurses and home support workers using knowledge of local geography, client needs, and continuity of care while following collective agreement guidelines and established planning procedures.
Key Responsibilities- Assign client visits based on client needs and continuity of care.
- Process client referrals and updates, respond to phone inquiries, and make scheduling changes as needed.
- Work with team members to ensure staffing levels meet client needs and quality service standards are met.
- Support clients, nurses, and home support workers to deliver quality care.
- Maintain confidentiality of client and work-related information.
