Job Title: Pharmacist

Job Summary:

We are seeking a highly skilled and experienced Pharmacist to join our team at the University Health Network. As a Pharmacist, you will play a critical role in ensuring the safe and effective use of medications in our healthcare setting.

Key Responsibilities:

  • Review and dispense medications to patients, ensuring accuracy and adherence to treatment plans
  • Monitor patients for potential medication-related problems and take corrective action as needed
  • Provide education and counseling to patients on proper medication use and management
  • Collaborate with healthcare teams to develop and implement medication therapy plans
  • Conduct medication reconciliation and ensure accurate medication lists
  • Participate in quality improvement initiatives to enhance patient care and safety

Requirements:

  • Completion of a Bachelor of Science in Pharmacy degree or equivalent
  • Active registration with the Ontario College of Pharmacists (Part A)
  • Experience working in a hospital or healthcare setting, preferably in a pharmacy role
  • Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams
  • Ability to work in a fast-paced environment and prioritize tasks effectively
