Job Title: Resource Consultant

We are seeking a highly skilled and compassionate Resource Consultant to join our team at the Alzheimer Society Southwest Partners. As a key member of our team, you will play a vital role in supporting individuals living with Alzheimer's disease and other dementias, as well as their care partners.

Job Summary:

The Resource Consultant will provide support and guidance to individuals and their families, connecting them with community resources and services. This role will also involve educating and modeling behavioral strategies for healthcare professionals to mitigate the risk of restraints in the Emergency Department.

Key Responsibilities:
  • Provide support and guidance to individuals and their families, connecting them with community resources and services.
  • Develop and maintain relationships with community agencies and organizations to facilitate referrals and coordinate client services.
  • Collaborate with healthcare professionals to educate and model behavioral strategies to mitigate the risk of restraints in the Emergency Department.
  • Assist with cognitive testing and assessments as needed.
  • Provide families with relevant information and resources to support their loved ones.
  • Liaise with primary care providers to support stabilization in the community.
  • Participate in coordinated care planning to ensure smooth transitions out of the Emergency Department.
Requirements:
  • University degree in social sciences or a related field.
  • Knowledge and understanding of community resources and services.
  • Experience working with individuals living with dementia and their care partners.
  • Ability to communicate fluently in French or other languages.
  • Proficiency with Microsoft Office Suite applications.
  • Valid driver's license and access to a vehicle.
