As a Support Worker, you will be an integral part of our interdisciplinary team, working closely with healthcare professionals to provide patient-focused treatment, rehabilitation, and support.

Key Responsibilities:
  • Provide care and support to patients according to their individual needs and care plans
  • Observe and report any changes in patient behavior, physical, or cognitive status
  • Perform delegated tasks and responsibilities as trained and authorized
  • Document patient progress and maintain accurate records
  • Respond to crisis situations and provide non-judgmental support
  • Collaborate with the healthcare team to develop and implement service plans
Requirements:
  • Certificate in Community Social Services or a related field
  • 2+ years of experience in a healthcare or social services setting
  • Current First Aid/CPR Certification
  • Knowledge of mental health issues and treatment options
  • Ability to establish rapport with patients and families
  • Strong observation and communication skills
