Job Summary

The Care Coordinator is a vital role within our organization, responsible for delivering exceptional patient experiences and supporting the smooth operation of our clinics. As a key member of our team, you will be the first point of contact for patients, providing a warm and welcoming experience that sets the tone for their care journey.

Key Responsibilities

  • Provide administrative support to the Care Coordinator Manager and Clinic Director, ensuring seamless communication and coordination of patient care.
  • Coordinate patient intake, data entry, and scheduling of appointments, maintaining accurate and up-to-date medical records.
  • Assist patients with completing paperwork, ensuring accuracy and minimizing errors.
  • Collect payment at time of service, ensuring timely and efficient processing.
  • Track coverage for all funding streams, ensuring approval and compliance.
  • Complete data entry and billing, maintaining accurate and up-to-date records.
  • Coordinate courier packages and process incoming and outgoing mail, email, and faxes.

Qualifications & Core Competencies

  • High School Diploma, G.E.D. or equivalent.
  • Excellent customer service skills and telephone etiquette.
  • High degree of organizational skills and ability to prioritize and multi-task.
  • Excellent communication (verbal/written) and interpersonal skills.
  • Strong computer and data entry skills.
  • Flexible and adaptable to change.
  • Experience in a healthcare setting an asset.
