About the Role

We are seeking an experienced Registered Nurse to join our Adult Team in Home Care as a Nursing Interventionist. In this role, you will be responsible for assessing, planning, and implementing care plans for clients requiring community-based services.

Job Summary

The RN Interventionist fosters continuity of care by liaising with primary care physicians, residential care facilities, and other programs/organizations providing services. You will work in multiple settings, including hospitals, clinics, and patient homes, to provide direct nursing interventions and promote health outcomes.

Key Responsibilities
  • Assess and plan care for clients with complex needs
  • Liaise with healthcare teams to ensure seamless care transitions
  • Provide education and support to clients and their families
  • Maintain accurate records and communicate effectively with stakeholders
Requirements

To succeed in this role, you must have:

  • Completion of an accredited nursing education program
  • Active or eligible registration with the College of Registered Nurses of Alberta (CRNA)
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification
  • 1-2 years of recent Home Care experience or equivalent combination of education and experience
  • Excellent interpersonal and communication skills
  • Ability to work independently and as part of a multidisciplinary team
Salary Information

This position is compensationed with a salary ranging from $35.87/hour to $43.82/hour, depending on experience and qualifications.
