About the Role

RhynoCare is seeking a highly skilled and compassionate Patient Care Specialist to join our team. As a Patient Care Specialist, you will play a vital role in providing close observation and support to patients who require a safe and caring environment.

Key Responsibilities
  • Observation and Support: Observe patients who pose a risk to their own safety or the safety of others, and provide close support to ensure their well-being.
  • Documentation and Communication: Maintain accurate documentation of patient observations and communicate effectively with the healthcare team to facilitate care planning.
  • Assistance with Daily Living: Provide assistance with activities of daily living, such as bathing, dressing, and grooming, under the close supervision of registered healthcare professionals.
  • Behavioral Support: Engage residents in meaningful activities, such as reading, storytelling, and photo albums, to promote cognitive stimulation and reduce behavioral risks.
  • Emergency Response: Respond promptly to emergency situations, such as patient elopement or violent outbursts, and notify registered healthcare professionals immediately.
  • Professional Image and Communication: Maintain a professional image and communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Education: Minimum Grade 11 or equivalent education as recognized by the Ministry of Education, Province of Ontario.
  • Experience: One year of recent related experience in patient care, hospital, long-term care, or complex continuing care setting required.
  • Language: Must be fluent in speaking and documenting in English.
  • Certifications: Current Non-Violent Crisis Intervention Certification and proof of current immunization for measles, mumps, rubella, varicella (MMRV), diphtheria, pertussis, and hepatitis B.
  • Police Check: Willing to undergo Vulnerable Sector Police Check as a condition of hire.
