Nurse Practitioner

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at St. Joseph's Healthcare Hamilton.
Job Summary:The successful candidate will work in collaboration with physicians, other members of the interdisciplinary team, and clients/families to provide evidence-informed and integrated care to registered outpatients of the TOPSS and SOC programs. The Nurse Practitioner will be responsible for providing clinical practice, leadership, research, consultation, and collaboration in a community-based setting.
Key Responsibilities:- Provide comprehensive assessment, diagnosis, and treatment of clients with schizophrenia and related psychotic disorders.
- Develop and implement individualized care plans in collaboration with clients, families, and healthcare teams.
- Prescribe and manage medications, as well as provide non-pharmacological interventions.
- Collaborate with community resources and supports to ensure seamless care coordination.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Current registration with the College of Nurses of Ontario in the RN-Extended Class with specialty certification as NP - Primary Health Care.
- Master's Degree in Nursing required.
- Proof of successful completion of controlled substances education required.
- Faculty appointment or eligibility for appointment with a School of Nursing in an academic institution.
- CNA certification in Psychiatric/Mental Health Nursing (CPMHN(c)) preferred.
- Demonstrated experience with psychiatric assessment and treatment.
- Providing pharmacologic and non-pharmacologic interventions.
- At least three years recent demonstrated experience as a Nurse Practitioner with experience in community-based settings servicing individuals diagnosed with schizophrenia and related psychotic disorders.
