About Personalized Support and Stabilization

Personalized Support and Stabilization is a community team of interdisciplinary professionals working together to provide intensive short-term support for individuals with complex health conditions, facilitating their safe transition from hospital settings back to their homes in the community.

Key Responsibilities:

  • Function under the operational supervision of the Manager or Team Leader and the work direction of a licensed designated occupational therapist (OT) or physiotherapist (PT) to work as a member of the healthcare team.
  • Provide assigned physical/occupational therapy therapeutic interventions.
  • Assist in modifying OT/PT care plans and facilitate transitions to community programs to enable clients to maximize their potential for independence.
  • Provide functional direction to family, report relevant observations of client's condition in writing, manage equipment safety, and inventory.

Qualifications:

  • Completion of a recognized Diploma level Rehabilitation Assistant Program with Occupational and Physical therapy components plus one year's recent, related experience working with the target population.
  • Valid BC Drivers License. Local area travel requires the use of a personal vehicle for which mileage will be reimbursed.
