About the Role
We are seeking a compassionate and skilled Physical Therapist to join our team as a Per Diem Home Health Therapist. As a member of our multidisciplinary health care team, you will provide rehabilitative care and skilled nursing to clients in their homes.
Key Responsibilities
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements to assess client needs.
- Implement individualized care plans to promote client health and independence.
- Educate clients and their representatives on rehabilitative care and activities necessary for independent living.
Requirements
- A current DE Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
