Community Mental Health Nurse

**About the Role**
We are seeking a highly skilled and compassionate Registered Nurse to join our team in providing exceptional care to Aboriginal clients with mental health concerns. As a Community Mental Health Nurse, you will work collaboratively with interprofessional teams to deliver person-centered care, focusing on recovery-oriented approaches and culturally sensitive practices.
Duties and Responsibilities
- Assess client needs and develop individualized care plans in consultation with healthcare professionals and community agencies.
- Provide counseling and therapy sessions to clients, either individually or in group settings, using various interviewing and assessment methods.
- Liaise with acute care hospitals, community mental health services, and other relevant agencies to ensure seamless transitions and continuity of care.
- Document assessments, diagnoses, interventions, patient responses, and outcomes according to professional standards and Fraser Health procedures.
- Aid clients in accessing medical appointments, hospital admissions, and social programs through provision of information, transportation assistance, and daily living support.
Requirements
- Graduation from an approved School of Nursing or Psychiatric Nursing.
- Two years' recent experience providing treatment services to individuals with mental health and/or concurrent disorders, including one year working with Aboriginal clients.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Valid Class 5 BC Driver's License and access to personal vehicle with appropriate insurance coverage.
