About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at peopleCare Communities. As a primary care provider for our residents, you will work collaboratively with our interdisciplinary team to deliver exceptional care and services.

Key Responsibilities
  • Provide comprehensive primary care to residents, including health assessments, diagnoses, and treatment plans.
  • Collaborate with physicians, nursing staff, and other healthcare professionals to ensure seamless care coordination.
  • Develop and implement person-centered care plans that address the unique needs and goals of each resident.
  • Lead and participate in quality improvement initiatives to optimize resident outcomes and healthcare services.
  • Provide education and support to residents, families, and staff on health promotion, disease prevention, and management.
  • Participate in research and evidence-based practice initiatives to advance the field of gerontological nursing.
Requirements
  • Nurse Practitioner Certification from a recognized program.
  • Current registration with the College of Nurses of Ontario as Registered Nurse in the Extended Class or RN (EC) and be in good standing.
  • Advanced knowledge and decision-making skills in assessment, diagnosis, and healthcare management.
  • Experience in Long-Term Care considered an asset.
  • Strong written and oral communication, critical thinking, care coordination, time management, and customer focus skills.
