Job Summary

Vancouver Coastal Health is seeking a Community Liaison Worker to join their team at the Raven Song Community Health Centre in Vancouver, BC. As a Community Liaison Worker, you will provide support services to clients with mental health and substance use issues, working across multiple sites and clinics within an integrated health service delivery model.

Key Responsibilities

  • Provide support services to clients with mental health and substance use issues
  • Work across multiple sites and clinics within an integrated health service delivery model
  • Monitor clients, document, and provide verbal reports on observations
  • Facilitate the use of community services and resources
  • Liaise and coordinate services with mental health and substance use professionals
  • Plan, organize, and facilitate groups as required
  • Act as an advocate for clients and provide information and support related to complex healthcare needs

Qualifications

  • Certificate from a recognized Health Care Worker program, plus two years' recent related experience
  • Valid driver's license and access to a personal vehicle for business purposes
