Job Summary

Vancouver Coastal Health is seeking a Registered Nurse to join the Transition Services team at Lions Gate Hospital in North Vancouver, BC.

Key Responsibilities

  • Receive and prioritize referrals for patients requiring home and community care services or transitional care.
  • Collaborate with the interdisciplinary team to provide continuity of care across the continuum.
  • Assess patients, liaise with team members, coordinate and implement discharge plans, and evaluate patient outcomes.
  • Establish and maintain therapeutic relationships based on respect.

Qualifications

Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years' recent related clinical nursing experience in an acute/community care setting.

Knowledge, Skills & Abilities

  • Comprehensive knowledge of BCCNM standards of practice and guidelines for registered nurses.
  • Ability to apply intensive case management principles within a patient and family-centered care model.
  • Knowledge of indigenous cultural safety, culturally safe principles, and practices of person and family-centered care.
  • Ability to engage in culturally competent and responsive care.
