We are seeking a dedicated Home and Community Physiotherapist to join our team in Toronto. As a vital member of our healthcare organization, you will be responsible for providing specialized physiotherapy care to individuals in the comfort of their own homes and within the community.

Key Responsibilities:

  • Provide physiotherapy services in the community to individuals of all ages
  • Work collaboratively with our team to ensure high-quality care and excellent patient outcomes
  • Develop and implement individualized physiotherapy plans to meet the unique needs of each client
  • Conduct thorough assessments and evaluations to identify areas of improvement and develop strategies for success
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination

Requirements:

  • Valid Driver's License (G2/G) and reliable insured vehicle
  • Current registration with the College of Physiotherapists of Ontario
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
