We are seeking a skilled Patient Care Coordinator to join our team, reporting to our Director of Clinical Services.

Job Description

The successful candidate will be responsible for:

  • Patient Flow Management: Develop and implement strategies to optimize patient flow, ensuring efficient use of resources and minimizing wait times.
  • Clinical Leadership: Supervise and support nursing staff to ensure high-quality patient care and adherence to established policies and procedures.
  • Quality Improvement Initiatives: Participate in quality improvement initiatives to identify areas for improvement and implement evidence-based solutions.
Responsibilities and Expectations
  • Provide Clinical Supervision: Supervise and support nursing staff to ensure high-quality patient care and adherence to established policies and procedures.
  • Coordinate Patient Flow: Coordinate daily patient admissions, transfers, and discharges to ensure efficient use of resources and minimize wait times.
  • Support Quality Improvement: Participate in quality improvement initiatives to identify areas for improvement and implement evidence-based solutions.
Skills and Abilities
  • Leadership: Demonstrated leadership skills, including the ability to prioritize tasks, manage multiple projects simultaneously, and motivate teams to achieve high-quality results.
  • Clinical Proficiency: Strong clinical knowledge and skills, with a focus on patient-centered care and evidence-based practice.
  • Communication: Excellent verbal and written communication skills, with the ability to effectively communicate with patients, families, and healthcare professionals.
