About the Role
We are seeking a highly skilled Clinical Pharmacist to join our team at Fraser Health Authority. This is a great opportunity for a motivated and experienced pharmacist to work in a fast-paced hospital setting, providing expert medication management services to patients.
Responsibilities
- To provide individualized pharmacotherapy and patient counseling, ensuring optimal medication use and minimizing adverse effects.
- To work collaboratively with physicians, nurses, and other healthcare professionals to develop and implement therapeutic plans, including medication management and monitoring.
- To assist in the development of drug therapy guidelines for specific disease conditions, using evidence-based practice and research.
- To prepare and submit written summaries and recommendations on drug usage to the Pharmacy & Therapeutics Committee.
- To participate in quality improvement initiatives, medication use evaluations, and clinical drug trials, staying up-to-date with the latest research and best practices.
Requirements
- Candidates must be registered as a Pharmacist with the College of Pharmacists of British Columbia and have completed an accredited Hospital Pharmacy Residency Program or equivalent experience.
- Broad knowledge of organization policies, procedures, and standards of care, as well as evidence-based clinical pharmacy practice, medication use evaluation, and research processes.
- Ability to communicate effectively, both verbally and in writing, and establish effective working relationships with colleagues, patients, and families.
- Physical ability to carry out the duties of the position, which may involve standing for long periods, lifting heavy loads, and working in a fast-paced environment.
