**Job Summary**

Kitchener Downtown Community Health Centre is seeking a highly skilled and compassionate Primary Care Clinician to join our Outreach team. This role will provide essential healthcare services to individuals who are homeless and precariously housed in Kitchener-Waterloo.

In this position, you will work closely with outreach clients, providing support to mobile primary care teams and participating in HART hub locations. You will also collaborate with other partners of the Inner City Health Alliance that serve the same clients.

You will be responsible for performing assessments, diagnoses, treatments, health education/counselling, and other clinical activities in accordance with the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (NPs) and all relevant provincial legislation and regulations.

The ideal candidate will possess a strong understanding of social determinants of health and a commitment to working within a client-centered, trauma-informed framework.
