We are seeking a skilled Nurse Practitioner to join our team at Sunset Country Family Health Team. As a key member of our primary care team, you will provide high-quality, patient-centered care to our community.
Responsibilities- Provide comprehensive primary care to patients, including health assessments, diagnosis, and treatment.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient outcomes and experience.
- Stay up-to-date with the latest evidence-based practices and guidelines in primary care.
- Licensure as a Nurse Practitioner by the province/territory of work.
- Experience in primary care or community health an asset.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
