We are seeking a highly skilled Registered Nurse to join our team in the Emergency/Medical/Hospice department at Fraser Health Authority. As a key member of our interprofessional care team, you will provide direct patient care, assess patients, and develop and implement individualized care plans.
Responsibilities- Provide direct nursing care to patients receiving emergency, medical, and hospice care, assessing patients and collecting information from various sources.
- Collaborate with patients and their families/caregivers to develop and implement care plans, promoting patient-centered care and advocating for patient rights.
- Participate in the care and discharge planning process, ensuring comprehensive care planning and delivery, and collaborating with the interprofessional care team.
- Coordinate nursing care, set priorities, assign and delegate tasks to support staff, and make referrals to the interprofessional care team according to established protocols and practice standards.
- Collaborate with members of the interprofessional care team, including physicians, licensed practical nurses, patient care coordinators, and other healthcare professionals/providers, to identify and resolve issues, adjust care plans as required, and address changes in patient condition.
- Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance to help patients achieve their healthcare goals.
- Assess learning needs of patients and families/caregivers, provide information and demonstration of care activities to support self-care and the discharge plan.
- Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Participate in nursing committees, care conferences, and councils as assigned or agreed to with the Manager.
- Participate in research and quality improvement activities by identifying patient care issues, collecting data, and identifying needs and recommendations for corrective action and changes to practices, procedures, or protocols.
- Participate in and attend in-services and other educational programs, identifying own learning goals and maintaining and updating current clinical competence and knowledge within the area of practice.
- Act as a preceptor or mentor to assigned new nursing staff and students, sharing information, demonstrating care activities, and participating in orientation.
- Provide feedback to the Manager/delegate regarding the effectiveness of patient care programs, policy/clinical decision support tools, and equipment.
- Perform other related duties as assigned.
Education and Experience:
- Graduation from an approved School of Nursing.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One year's recent experience providing emergency nursing care in an acute/rural hospital setting.
- Current certificate in CPR.
Skills and Abilities:
- Current cardiac monitoring skills.
- Current emergency assessment and medical assessment skills.
- Ability to determine and deal with emergency situations and make appropriate clinical decisions.
- Ability to initiate intravenous therapy and maintain I.V. lines.
- Ability to complete initial and ongoing patient assessment (clinical and diagnostic reasoning) and provide nursing care through appropriate/prescribed technical, therapeutic, safety type interventions.
- Ability to use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence/evidence-informed practice.
- Ability to communicate effectively and professionally with patients, families/caregivers, the public, medical staff, and members of the interprofessional care team using verbal, written, and electronic communication means.
- Ability to promote person-centered care that demonstrates empathy for patients and families/caregivers, sensitivity to diverse cultures and preferences, and patient advocacy.
- Ability to plan, organize, and prioritize work.
- Ability to integrate and evaluate data from multiple sources to problem-solve effectively.
- Ability to collaborate with and promote cooperation among members of the interprofessional care team.
- Ability to teach and instruct patients and families/caregivers about topics essential to their health care and well-being.
- Ability to operate related equipment, including non-invasive blood pressure machines, pulse oximeters, tracheostomy devices, and computers and related software.
- Physical ability to perform the duties of the position.
