About the Role

Bayshore Home Care Solutions is seeking a passionate and caring Registered Practical Nurse (RPN) to join our team in the Delhi area of Norfolk County. As a community nurse, you will provide high-quality home health care to clients in their own homes, utilizing your nursing skills to ensure their comfort and safety.

Key Responsibilities

  • Provide nursing care to clients in their homes, including palliative, pediatric, wound care, and health teaching
  • Develop and implement individualized care plans to meet the unique needs of each client
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Teach and delegate tasks to clients and their families, promoting independence and self-care

Requirements

  • Registered Practical Nurse (RPN) with a valid license and current CPR certification
  • Experience with tracheostomy and ventilator care an asset
  • Valid driver's license and reliable vehicle
  • Updated immunization records and clear criminal background check
