Job Description

Role Summary

We are seeking an experienced Palliative Care Social Worker to join our team at Surrey Memorial Hospital. As a key member of our interdisciplinary team, you will provide clinical social work practice and support to patients and their families facing complex palliative and bereavement issues.

Your primary responsibility will be to assess psycho-emotional and social needs, develop comprehensive therapeutic counseling services, and collaborate with the interdisciplinary team to coordinate care plans. You will also participate in research activities, program planning, and development to improve patient outcomes.

In this role, you will have the opportunity to work closely with patients, families, community supports, and healthcare professionals to address the unique challenges faced by those affected by life-limiting illnesses. Your expertise and compassion will make a significant difference in the lives of our patients and their loved ones.

Key Responsibilities:

  • Assess psycho-emotional and social needs of patients and their families
  • Develop and implement comprehensive therapeutic counseling services
  • Collaborate with the interdisciplinary team to coordinate care plans
  • Participate in research activities, program planning, and development
  • Provide education and consultation to patients, families, and healthcare professionals
  • Maintain accurate records and reports

Requirements

To be successful in this role, you will possess a Master's Degree in Social Work from an approved school of Social Work. You will also require two years' recent, related clinical experience working with hospice palliative patients and their families, or an equivalent combination of social work education, training, and experience.

You must be registered with the BC College of Social Workers and hold a valid BC Driver's License. Additionally, you will need to demonstrate excellent communication and interpersonal skills, as well as the ability to work effectively in a multicultural team environment.
