Fraser Health is seeking a dedicated and compassionate Clinical Care Specialist to join our team in Surrey, B.C.
The ideal candidate will have recent relevant clinical experience in surgical services and be able to provide person-centered care to adults and children in an inpatient unit and/or outpatient clinic or setting.
This role is responsible for providing and coordinating pre and post-operative nursing care, including assisting with surgical and interventional procedures and discharge teaching.
We are looking for someone who can work collaboratively with the inter-professional care team and promote a culture of respect, caring and trust.
Key Responsibilities- Assess patients and collect information from various sources to determine the best course of action.
- Make nursing diagnoses and develop care plans that address the patient's needs.
- Provide and coordinate pre and post-operative nursing care, including assisting with surgical and interventional procedures.
- Teach patients and families about illness prevention, health restoration and health maintenance.
- Collaborate with the inter-professional care team to ensure comprehensive care planning and delivery.
